Document Checklist
1. Application Form
Must be completely filled and signed by the applicant.
2. Passport-size Photograph
One recent color photograph with a white background.
Size: 3.5 cm x 4.5 cm.
Do not staple to the application form.
3. Passport/Travel Document
Must be valid for at least 3 months after the intended stay.
Issued within the last 10 years.
Must have at least 3 blank pages.
4. Overseas Travel Medical Insurance
Must cover all Schengen countries.
Minimum coverage: €30,000 for medical emergencies, including repatriation.
Coverage period: At least 91 days from the date of travel.
5. Confirmed Travel Itinerary
Proof of travel plans, including intra-Schengen flight reservations if traveling to multiple Schengen States.
6. Invitation Letter from Employer
Official letter from the employer in Malta.
Must include:
Professional status.
Work details in Malta.
Signed by an authorized representative with name and designation.
7. Curriculum Vitae (CV)
Updated CV of the applicant.
8. Reference Letter from Past Employer
A signed reference letter from the applicant's previous employer.
9. Original Bank Statement
Showing financial transactions for the last 6 months.
Must be stamped and signed by the issuing bank.
10. Employment Contract
Signed employment contract.
Certified as a true copy by a Maltese Public Notary.
11. Approval in Principle Letter
Confirmation from Identita regarding the work permit request.
12. Proof of Skills/Qualifications
Original and apostilled proof of qualifications if required for the job. Examples include:
IELTS Level 3 certificate.
Certifications from relevant institutions (e.g., medical credentials).
13. Proof of Accommodation
Any of the following:
Confirmed hotel booking.
Lease agreement.
Proof of stay from an invitee (with a signed declaration and a copy of the invitee’s ID card).
If traveling to multiple Schengen countries, provide proof for each destination.
14. Additional Documents (If Required)
Copy of the host’s ID and invitee’s passport bio-data page.
Proof of residence:
Title deeds, rental agreements, or utility bills.
Proof of income:
Salary slips, pension details, or official income statements.
Health insurance policy details.

Applications with forged or falsified documents will be refused.
Non-Refundable Fees:
Visa fees will not be refunded in case of rejection.
