Document Checklist
1. Passport
Issued within the last 10 years.
Valid for at least 3 months after departure from Schengen.
Contains at least two blank pages.
2. Visa Application
Form: Fully completed and signed twice by the applicant (signature must match the passport).
Photos: Two recent passport-sized color photos (white background, no older than 6 months).
3. Flight Itinerary
Reservation only (indicating first entry date and location).
4. Travel Medical Insurance (TMI)
Issued by an approved insurance provider.
Minimum coverage of €30,000.
Valid across Schengen countries.
Covers emergency medical and repatriation expenses.
5. Documents Confirming Trip Purpose
Work Permit: Original and a copy.
Employer Registration Proof: KRS (National Court Register of Poland).
Employment Letter:
Issued on official company letterhead.
Must include employer’s address, contact details, applicant’s role, salary, and duration of employment.
Details on accommodations (if applicable).
Transfer Letter: If applicable (for international transfers).
Supporting Documents (Optional): Professional/educational certificates, work experience details, and Police Clearance Certificate.
6. Accommodation Proof
Proof of accommodation (lease agreement or booking confirmation).
7. Additional Documents
Any other documents relevant to the visa application.
8. Residence Proof
Proof of legal residence within the Embassy/Consulate jurisdiction.
9. Passport Data Pages Copy
A copy of the first and last data pages of the passport.
Copies of previous/current Polish or Schengen visas (if applicable).
 
Additional Notes
All documents must be in Polish or English.
Items must be arranged in the order listed above.
The passport will remain with the Embassy/Consulate during processing.
 
Important Reminders
The Embassy/Consulate may request additional documentation or a personal interview.
The standard processing time is 15 days but may extend to 30 days in some cases.
